The UCSF Fresno Department of Medicine residency program offers a three-year categorical ACGME-accredited program. 

The UCSF Fresno Department of Medicine residency program participates in the National Resident Matching Program (NRMP).  All applications are handled through the Electronic Residency Application Service (ERAS) sponsored by the Association of American Medical Colleges.

ERAS

UCSF Fresno participates in program signaling through ERAS. For the 2025-2026 ERAS application cycle, program signals will be fully integrated into the ERAS application. We receive thousands of applications and wish we could invite all applicants to interview with us. In addition to holistic review aligned with our program mission, program signals will be used to help us gauge interest when making our initial decision to invite an applicant to interview.

Application Requirements

  • Graduation from medical school by June.  For international medical graduates, we prefer a graduation date by early May.
  • 3 letters of recommendation (LOR), with at least one being a clinical LOR from a Department of Medicine faculty member .  We also encourage a department chair’s summary letter of evaluation when available.
  • Pass for either USMLE or COMLEX.  We do not require a Step 2 score to interview or rank.
  • We require all international medical graduates that have not completed rotations in the United States to complete at least 4 weeks of observerships in at least 2 different clinical settings. Hands-on experience is preferred.  We will consider international graduates who are applying less than 5 years since graduation.

Holistic Review Selection Process

Our Department of Medicine residency program utilizes a holistic review of all applicants who apply to our program.  A holistic review allows us to have a flexible, individualized process that gives a balanced consideration to each applicant’s experiences and attributes.  Under this framework, each candidate is evaluated by criteria that are mission-driven and specific to the needs of our institution and the community we serve. No one metric or academic credential is used to assess the candidate pool.  Our goal is to ensure that each of our residents bring their unique experiences and skills to positively add to our learning climate.

Interviews

All interviews will be virtual.

We will host virtual happy hours throughout the season to give applicants an opportunity to get to know our residents.

Post-Interview Communication

As a program, we understand that the interview season is stressful and time-consuming.  We want to both respect the time of all of our applicants and ensure clear and transparent communication.  As such, we have developed the following policy around post-interview communication:

  • We discourage routine thank-you notes or e-mails.
  • We are eager to answer factual questions about the program.
  • We do not use post-interview emails or letters of intent, nor the absence of communication, to influence an applicant’s ranking.

Minimum Employment Requirements

To be eligible for a residency/fellowship appointment with UCSF Fresno, all current or expected trainees must possess one of the following for the duration of their anticipated years of training:                                                                                                                                       

be a United States citizen  

be in possession of a valid United States Permanent Resident Card (Green Card)  

be in possession of a valid United States Employment Authorization Document (EAD) Work Permit  

be in possession of a valid ECFMG-sponsored J-1 visa* 

UCSF Fresno does not generally sponsor H-1B visas. Please contact individual programs regarding their policy on H-1B visa sponsorship.  

*UCSF Fresno does not accept dental applications from graduates of non-U.S. or Canadian dental schools. Dental applicants are not eligible for ECFMG sponsored J-1s. 

Trainees in programs rotating to the VACCHCS must follow Federal Law and register for Selective Service. Trainees may not be eligible for employment in programs that rotate to the VACCHCS if they have not registered for Selective Service or obtained a Status Information Letter from Selective Service indicating they were either not required to register with Selective Service or are exempt from the registration requirement. The determination to train at a VA facility is at the discretion of the VA, additional information regarding Selective Service can be found at: Selective Service website. Trainees are expected for follow VACCHCS’ Background Check process, more information can be found at VACCHCS’ Background Check

Employment is contingent upon trainees clearing a UCSF Employment Misconduct Disclosure review where you will be required to disclose any final administrative or judicial decisions within the last seven years determining that you committed any misconduct and provide information related to investigations and appeals. Accordingly, UCSF may revoke its offer or appointment, or amend the conditions thereof, based on the screening results and/or signed attestation, including any required written explanation of any “yes” response on the attestation statement. California Medical License

Trainees are required to comply with all State of California medical licensure laws.    


Background Checks

UCSF Fresno is committed to selecting and hiring the most capable house staff to pursue its strategic goals of excellence in teaching, learning, research, scholarship, creative activity, and service to the community. All offers of appointment to UCSF Fresno sponsored GME programs are specifically conditioned upon successful completion of a background check. Background checks will be carried out by one or more agencies on behalf of the UCSF Fresno GME Office. An additional independent background check may be conducted by an affiliated training site as a condition of rotating/onboarding at that facility.  

Trainees rotating at VACCHCS are expected for follow VACCHCS’ Background Check process, more information can be found on the VA Affiliation Agreements website. 


Information for International Medical Graduates

Graduates from medical schools outside the United States (including Canada) must be certified by the Educational Commission for Foreign Medical Graduates (ECFMG) prior to enrollment into an ACGME-accredited program.
