Community Regional Medical Center (CRMC) is part of the largest and most comprehensive hospital system in the San Joaquin Valley. Community Medical Centers (CMC) is a nonprofit, locally owned health care system that operates three acute-care hospitals and a variety of other health care facilities.

The Regional Medical Center provides inpatient and comprehensive outpatient services to patients of all ages. This 733 bed hospital offers a full range of inpatient services. Special hospital services include the only level one trauma center in the 15,000 square-mile service area between Sacramento and Los Angeles, a regional burn center, and the largest Level III neonatal intensive care unit in the Valley. The Regional Medical Center has one of the busiest birthing centers in California. It is the only hospital in the area where recovering mothers and critically ill babies can stay together for the treatments they need.

CRMC has a management services agreement with a network of primary care doctors and is active in managed care, computerization of medical records, and quality improvement/best practice efforts. Community has implemented Epic electronic health records (EHR) system in its hospitals and for more than 120 physicians in the Fresno/ Clovis area.

The Regional Medical Center has a strong partnership with UC San Francisco’s Fresno Medical Education Program. The administrative offices of the UCSF Fresno Medical Education Program are located on the CRMC campus.

The CMC Family Health Center is located in the Deran Koligian Ambulatory Care Center. The Family Health Center consists of 15 exam rooms each equipped with a wall-mounted computer in order to easily review labs and other studies with patients. In addition, there is a large resident conference room equipped with several computers and an AV system used for teaching. The ACC medical office building borders Community Regional Medical Center and the UCSF Fresno Center. In addition to the family medicine center the new, two-story building includes Women’s Health, Children’s Health, Internal Medicine, HIV Special Services, Ophthalmology and Dental services. CMC residents have easy access to rotations and lectures taking place at CRMC or the neighboring UCSF Fresno Center.

CMC residents have the opportunity to care for many patients with a wide spectrum of clinical pathology from diverse cultural backgrounds. This includes Hispanic and Southeast Asian ethnic groups, as well as African Americans, Native Americans, and others. A bilingual staff and translation services facilitate access to health care for the area’s diverse population.

Family medicine faculty based at CMC are quite experienced and have skills in inpatient care, HIV/AIDS management, Hepatitis C, and research methodology. Faculty from the community regularly precept residents and medical students at the CMC Family Health Center. Many of these preceptors are graduates of the UCSF Fresno family medicine program and are in private practice or in community health centers and other practice settings in the community. They are eager to share information with residents about the many exciting practice options available to family physicians after graduation

United Health Center – Parlier (UHC) is located in Parlier, a predominantly Hispanic, agriculture based, rural community. Supervision comes from core UCSF Fresno faculty.

UHC – Parlier is located in a Federally Qualified Health Center facility located thirty minutes from Community Regional Medical Center (CRMC). Our newest pathway site, established in 2017, the center includes six exam rooms, a resident work area with library and online resources, a modern laboratory with a full-time technician, x-ray services, as well as pharmacy and ancillary services. Patients requiring inpatient medical services are admitted to Adventist Health Center – Reedley or CRMC.  This pathway established its relationship with the residency program in 2015, with our residents providing prenatal care.

Serving a primarily Hispanic population, translation services and bilingual staff facilitate access to care at the federally qualified health center (FQHC). UHC is committed to meeting the health care needs of the community.

The UHC pathway offers residents an excellent education that incorporates community-oriented primary care. UHC residents have a diverse patient population with ample opportunities for prenatal care, deliveries, post-partum care and outpatient procedures. The acquisition of ambulatory care skills in a multicultural setting is also emphasized providing a practical and relevant education.

The variety and demographics of the population served by this standalone FQHC make it an attractive prospect for the residency with a rich cultural and patient care demographic.  This particular pathway site also engages significantly with the surrounding community, including extensive work and outreach with seasonal farmworkers which would provide fantastic additional opportunities for residents to participate in activities to address social determinants impacting the health of their patients.
